We offer monthly subscription plans (Essential Wellness, Premium Care, Elite Concierge) at published rates. All prices are in USD. Prices may change with 30 days' notice; existing subscribers retain current pricing through the end of their billing cycle.
Subscriptions are billed monthly in advance to your credit/debit card on file. Co-pays, deductibles, and non-covered services are billed separately. You authorize us to charge the payment method on file for all applicable fees.
